A BMI in the 18.5 โ 24.9 range is associated with the lowest risk of weight-related chronic diseases. BMI above 30 is associated with significantly elevated risk of type 2 diabetes, cardiovascular disease, hypertension, sleep apnea, and certain cancers.
Adjusted BMI Thresholds for South Asian and Asian Populations
Standard WHO thresholds were developed primarily from data on European populations. Research has shown that South Asian, East Asian, and Southeast Asian populations face increased health risks at lower BMI values due to differences in body fat distribution โ particularly higher rates of visceral (abdominal) fat at equivalent BMI scores.
For these populations, many health authorities โ including India's National Institute of Nutrition and the WHO expert consultation โ recommend lower action points:
Overweight: BMI โฅ 23 (instead of 25)
Obese: BMI โฅ 27.5 (instead of 30)
This is particularly relevant for Indian, Chinese, Japanese, Korean, and South-East Asian populations.
BMI and Health Risks โ What the Research Shows
Multiple large-scale studies confirm the relationship between BMI and health outcomes. Critical Limitations of BMI โ What It Cannot Tell You
BMI is a population-level screening tool, not a diagnostic test. It has well-documented limitations that you should understand before acting on your BMI result:
- Does not distinguish muscle from fat: A professional athlete or bodybuilder may have a BMI of 28-30 (classified as overweight or obese) despite having very low body fat and exceptional cardiovascular health. BMI treats muscle and fat identically.
- Does not measure fat distribution: Visceral fat (around internal organs) is far more dangerous than subcutaneous fat (under the skin). Two people with identical BMI can have very different fat distribution patterns and health risks. Waist circumference and waist-to-hip ratio capture this risk more accurately.
- Age-related changes: As people age, body composition shifts โ muscle mass decreases while fat increases. An older adult with a "normal" BMI may actually have higher body fat than a younger person with the same BMI.
- Sex differences: Women typically carry more body fat than men at any given BMI due to physiological differences. A BMI of 24 in a woman may represent more body fat than the same BMI in a man.
- Ethnic population differences: As discussed above, the same BMI indicates different levels of health risk depending on ancestry and body composition patterns.
